HOW TO REQUEST IRS FORMS 1095-B

As permitted by federal law, RYKA, INC dba Meyer Pharmacy is no longer automatically mailing federal Forms 1095-B.  If you wish to receive a copy of your Form 1095-B, you may request one by contacting Carrie at carrie.s@meyerpharmacy.com

Your request must include your first and last name, address, your contact number and e-mail address (if you want an electronic copy of Form 1095-B). You should expect to receive your Form 1095-B within thirty (30) days of your request.
